Insurance


I am in-network with Cigna Behavioral Health, Mines & Associates, ComPsych, Health Management Systems of America (HMSA), MHN, Humana (LifeSync), APS Healthcare and Blue Cross/Blue Shield of Arizona

 

Please check your policy for questions on deductible and co-pay amounts, and whether pre-authorization is necessary.  Co-pays are due at the time of service.  No credit options exist.  Clients are responsible for all fees for services not covered by managed care or insurance. 

When making an appointment, please provide your date of birth, Member ID number, Employer Group/Account number, and Authorization Number (if necessary) so that I may call to learn about your insurance company's specific provider requirements. 

 

Clients are often unaware that a company other than the one issuing their health insurance card may cover the behavioral benefits.  For example, Blue Cross/Blue Shield subscribers who are employed by the City of Phoenix or Federal Government likely have Biodyne as their behavioral health insurer.  While I am a BC/BS provider, I am not in-network with Biodyne (a Magellan company).  You may still see me utilizing your BCBS benefits, but co-pays and out-of-pocket expenses will be different.

If your behavioral health benefits are associated with a different company, ask  your insurance provider if you have Out-of-Network Benefits to determine if you can be reimbursed for a portion of my fees.  I will provide you with the necessary recepits and information in the form of a "Super Bill" to allow you to gain reimbursement if allowed by your provider.

 

If you have a Flex-Spending or other pre-tax health spending account, you can request reimbursement according to your employer's policy and procedure.  Some health spending accounts will pay providers directly while others will only reimburse the employee.  If your plan pays the provider directly, I will wait for payment provided you bring documentation showing how your plan works.  Otherwise fees are due at time of service.

Private Pay

 

Initial Phone Consultation: No charge (~15 minutes)

Standard Sessions: $90 (50 Minutes)

Extended Sessions: $110 (75 Minutes)

 

Forms of Payment

 

Cash, personal checks, Visa, MasterCard and PayPal are accepted for payment.  

 

Late Cancelation/Missed Appointment Policy

 

In order to avoid a late cancelation/missed appointment charge of a $25, clients must provide 24 hours notice if they know they can't make a scheduled session.  Failing to notify the counselor will result in a charge, except in extreme cases of serious illiness, unavoidable circumstances or catastrophe.  Reasonable documentation is accepted as proof (medical bills, invoice for auto repair, etc.).  Insurance will not reimburse for missed appointments. 

 

Frequently missing and canceling appointments on short notice is detrimental to your progress and endangers the working relationship between client and therapist.  Because I am in private practice part-time, my schedule of availability may not be the best match for every client.  When cancelations and missed appointments suggest that a client may need a different schedule than I can provide, clients will be supported in transitioning to a therapist with day or afternoon scheduling options.
